Surgical Treatment of Giant Hemangioma of the Liver

Abstract

|
Background: The management of giant hemangioma of the liver is still controversial. This study was to investigated the role of surgical treatment.
Methods: Nine patients with giant hemangioma of the liver was treated surgically from Jan.1990-Dec. 1998. The authors reviewed the results of surgical treatment retrospectively.
Results: The 9 patients had mean age of 47, with a male to female ratio 1:3.5. The most common symptoms were discomfort or pain on the right upper quadrant and epigastric area. But 2 patients was asymptomatic on the time of diagnosis. On physical examination 3 patients had right subcostal tenderness, epigastric tenderness, palm-sized palpable mass each other with no specific findings on other 6 patients. Preoperatively 4 patients were diagnosised as hemangioma but 5 patients as hepatic tumors. Diagnostic biopsy was performed on 2 cases with one significant hemorrhage in laparoscopic biopsy. The lesions were located at either of left or right lobe of liver but slightly more at left. The mean size of tumor was 9.5§¯ ranging from 5 to 20§¯. Operative methods were liver resections including lesions in eight cases except improperly diagnosed case as a cystic liver disease. There was no major complications.
Conclusion: With advance of operative facility, liver resection can be the surgical treatment of choice for giant hepatic hemangioma without significant morbidity.
